I __________________________, hereby retain the services of Alfa Transportations, hereinafter “Limo or Bus Service” in accordance with the terms set forth in this document:
FEES:
A. I understand and agree that all deposits are NON-refundable.
. I understand and agree that I shall pay Limo or Bus Service the entire sum of my charter should I cancel within seventy-two (72) hours of the start of my charter.
I understand and agree that I shall pay 20% of the sum of my charter should I cancel this reservation.
C. I understand and agree that I shall pay an overtime rate per hour that shall be applied beginning 15 minutes subsequent to the end of my scheduled charter. I understand and agree that such sums shall be in addition to all monies paid to Limo or Bus Service previously.
D. I understand and agree that all balances are to be paid to Limo or Bus Service employee and/or agent prior to the beginning of my charter.
MECHANICAL FAILURE:
E. I understand and agree that Limo Service is not liable to my party in the event of mechanical breakdown while my party is on charter and will only be responsible for making up lost time at a mutually agreeable date.
DAMAGE TO LIMO SERVICE PROPERTY:
F. I understand and agree that I am fully financially liable for any and all damage to Limo or Bus Service vehicle(s) caused by any and all members of my party.
G. I understand and agree that smoking is strictly prohibited in any and all of Limo or Bus Service vehicles. Any violation of said prohibition shall result in a sanitation fee of $350.00.
H. I understand and agree that carpet/seat burns shall be payable at $750.00 per incident, and I further understand and agree that a sanitation fee of $350.00 shall be applied if Limo or Bus Service deems such service necessary.
I. I further understand and agree that if I am making this reservation on another party’s behalf, I am agreeing to be financially liable for any and all damage to Limo or Bus Service Vehicle(s) caused by said party.
CONTROLLED SUBSTANCE ABUSE AND ALCOLHOL USE BY MINOR(s):
J. I understand and agree that Alcohol Consumption by a minor in the State of California and use of any controlled substance by any member of the party using Limo or Bus Service services is prohibited by law, and shall result in the immediate termination of Limo or Bus Service services.
a. I further understand and agree that I will be liable for payment of any and all fines assessed to Limo or Bus Service as a result of said behavior.
RIGHTS AND RESERVATIONS:
K. I understand and agree that Limo or Bus Service’ employees and/or agents have the right to terminate my charter without refund and without prior notice whenever it is deemed that there has been a violation of company policy due to my party’s blatant indiscretion with regard to safety, public policy, and/or company policy.
a. I further understand and agree that such blatant disregard includes but is not limited to standing through the sunroof, public nudity/indecent exposure, abuse of Limo or Bus Service’ property, employees, and/or agents, and any violation of paragraph D of this agreement.
L. I understand and agree that Alfa Transportations is not responsible for delays or the termination of the charter due, but not limited, to earthquake, fire or inclement weather conditions, public roadway closures, vehicle accidents, and/or traffic.
M. I understand and agree that Limo or Bus Service and or their employees/agents are not responsible for any and all articles left by my party in Limo or Bus Service vehicle(s).
N. &nb sp; I understand and agree that Limo or Bus Service vehicles cannot and will not be loaded beyond the seating capacity for each vehicle.
This document sets forth in writing that you have signed indicating your complete understanding and agreement with regard to your retention of Limo or Bus services. Terms pertaining to fees are contained within the provisions of this letter document. There are no side agreements or oral understandings that modify this document. Any modification of this document will be valid only if contained in a writing signed by each of us. If you have any questions when you read this document, please be sure to raise and discuss all such questions with us.
I ___________________________________ have read the terms, discussed and agreed to the same, and by doing so, I also acknowledge receipt of a copy of this document. Additionally, I hereby authorize Alfa Transportations to charge my credit card (or debit card) for the entire sum of the charter and authorize any future charges to cover any and all incidentals that may occur.
